Click to download the following two documents. Note: The Report of Health Examination needs to be filled out by a parent/guardian and a health examiner.
Step 2:
Create a login
Click on the link below and follow the instructions to create a private parent account.
Step 3:
Submit application
Complete the application and provide all the items noted in the Checklist, as well as the Report of Health Examination for each student applying.
